Job Description

An exciting opportunity for a healthcare/social services registered professional to work as a Navigator in our Auckland based Supported Living service. The role of the navigator is to provide safe and supportive interventions to clients both in the prison and on their release to the community.

You will work closely and collaboratively with tangata whaiora assisting to identify, describe and work towards goals which link them back to their communities of choice. You will also work in partnership with the service user and appropriate Corrections staff to deliver a tailored Reintegration Service Plan.

The role is predominantly Monday to Friday 8.30am - 5pm, however you will have to be flexible to the needs of any individual who may require support outside of these hours.

What you need

Essential skills:

  • Registered with a Professional Body covered by HPCA 2003 Act. A RN, OT or Clinical Psychologist/Psychologist or
  • Be a Registered Social Worker with the Social Work Registration Board (SWRB)
  • Hold a current APC
  • Relevant health care/social services degree level qualification
  • Full clean driver's licence and eligible to work in New Zealand
  • Three or more years of service delivery in a health/social service setting
  • Strong English language communication skills - written and verbal

Desirable skills:

  • An understanding and genuine interest in working with the relevant client group
  • Basic level of computer literacy e.g. email, Client Management systems, internal intranet
  • The ability to use a variety of skills to engage people in a wide range of situations to achieving quality client outcomes
  • Experience working in Mental Health/Addiction/Intellectual Disability/ Corrections or relevant social services sector
  • Competence and experience in effective engagement with community agencies and networks
  • Knowledge of community resources and support networks
